This will begin a request to delete your account for a mobile app managed and hosted by the Authoritek Development group.

Process:
 

 1. Once this form is filled out, we will contact account owner and verify they would like their account to be deleted

2. Once verified, we simply delete all data tied to their authentication login, along with the actual login account itself

3. Once deleted, the account cannot be recovered, and all of their personal information will be removed from our database including any accrued reward points
